District Nurse

As part of our Integrated GAC Care Team, you’ll manage a community caseload across local West Sussex and Hampshire using your clinical expertise in Nursing to deliver safe, compassionate care.

You’ll coordinate referrals, complete holistic assessments, and maintain person‑centred care plans to the highest professional standards. Working with autonomy and accountability, you’ll help prevent unnecessary hospital admissions, support timely discharges, and contribute to re‑enablement, rehabilitation and health‑promotion programmes.
