We’re looking for candidates with those skills to join us, working at Eastbourne District General Hospital.
This is an exciting development opportunity for someone who is organised, caring and has a good eye for detail. The JCR In Reach service is a new element to JCR and has recently received funding to be made a permanent team. The small team are based in the hospital and attend various meetings to help identify the right people to go home from hospital with JCR.
JCR is a service that supports individuals to regain their independence following an accident or a period of illness, and we help them to remain in their own home. The person will have goals to achieve at home over a short time-frame and JCR supports rehabilitating them to ideally get back to how they were before the accident or illness so they can live their life as they wish.
Your role is to work in a fast-paced environment and be willing to communicate calmly and professionally with others to help identify suitable people ready to go home with JCR. You will also be liaising with family / Next of Kin on the persons behalf, with their consent.
As a result of this role, you will build up good relationships with various professionals in the hospital and will be working closely with a Team Leader.

Driving licence and access to a vehicle or be able to fully demonstrate the travelling requirements of the post

Note: The working pattern of the role is predominantly 8:30am-5:00pm Monday to Friday but you will be part of a rota where occasional working at the weekend is required.
